Charity Mini Golf Tournament

Think you’re the master of mini golf? Try your luck at the 1st Annual Relay for Life Team Lucky Stars Mini Golf Tournament! Teams of two can compete for a chance to win trophies, cash and prizes, as well as individual prizes. All proceeds from the event will benefit Relay for Life Team Lucky Stars.

Registration
Registration for the tournament is $25 for adults and $10 for kids 12 and under until August 1. Your registration includes a T-Shirt and one entry into the door prizes. Registration will increase by $5 after August 1 and may not include a T-Shirt. 

Complete the attached registration form and send to Team Lucky Stars via mail or drop off at Red Carpet Golf and Recreation Center in Washington, PA. We can only accept cash, check or money order at this time.

Longest Drive & Hole-in-One Contest, Chinese Auction, 50/50 & Concession Stand
Contests are $2 per chance or 3 chances for $5 with unlimited entries. Chinese Auction and 50/50 tickets are 3/$1 or an arm length for $5. Celebrity Raffle chances are $5 each or 3/$10.
