You are required to fill this form out and return it to your employer.
If you work, then listen up: All employees are required by Pennsylvania law to fill out a new Earned Income Tax form and return it in to their employers.
While the forms should be available through your employer, you can also print out the attached PDF.
Remember: Filling out this form is mandatory.

And one more tidbit: If a form is not completed, and your taxes are sent to the the wrong municipality or school district, you will still be responsible for paying those taxes.
