Do You Qualify for the Senior Life?

Learn about the LIFE program and healthcare solutions for Seniors Living at Home. Senior LIFE is a program funded my Medicare and Medicaid designed specifically to sustain independence among seniors by providing and/or coordinating a comprehensive range of support services all through a single network. 

For those who qualify for Medicare and Medicaid there is no cost to join Senior LIFE. The eligibility requirements are 55 years or older, resides in one of the areas served by Senior LIFE, determined medically eligible and able to be safely served in the community. Senior LIFE's services include Physician services, Nursing care, Medication monitoring, Therapy, Social Services, Home care, and Transportation. A Senior LIFE care plan is customized to the individual needs of each person. Our goal is to improve the member's quality of life and maximize independence in a home environment. Register for this program online , at the library's circulation desk, or call 724-941-9430.
